How to use this page

  1. Start with the numbers someone else in your household might not have saved. The sheet has space for emergency services, health and care, home services, and family contacts.
  2. Confirm each number from a source you trust before writing it down. Include the area code and label whether a number reaches an office, an after-hours line, or an individual.
  3. Put the sheet near the front of your binder. Review it after a move, a change of provider, or a new school year. Keep medical details and other sensitive records elsewhere.

A simple example

Fictional example: write “HVAC service — [company name and verified number]” in the Home section. In Notes, add “Service agreement kept in the filing cabinet.” The reference tells someone where to look without storing private account details.

What if a contact changes?

Replace the page or cross out the old entry clearly. A short, current list is easier to use than several pages of outdated numbers.
